  • Use this form to register one or more students for vocal or instrumental lessons arranged through Enact Music. You may register up to three students using this form, identified as Student 1, Student 2 and Student 3. To register more than three students, please complete and submit this form again for the additional student or students.

    Where a student is under 18, the form must be completed by the student’s parent or legal guardian.

    We collect and use the information provided to process your registration, identify a suitable independent tutor, arrange and administer lessons, manage payments and communications, and meet our safeguarding and legal obligations.

  • Acknowledgements

  • If a student becomes ill or injured, Enact Music or the tutor may provide appropriate first aid, contact the parent, legal guardian or emergency contact, call the emergency services and share relevant information with those involved in the student’s immediate care.

    Where a student has been prescribed emergency medication, such as an asthma inhaler or adrenaline auto-injector, the parent, legal guardian or adult student is responsible for ensuring that it is available, in date and brought to every lesson or activity.
