2024 Summer Field Trip Registration

Your child has won the opportunity to go on a field trip by performing exceptionally well in this summer's science fair! Please read the information at the top of this form then fill out the permission slip by July 8th, 2024

Location:  South Central Extension Community Office, 5040 S Central Ave., Phoenix, AZ 85041 
Pickup will be at South Point Junior High School.
Cost: No cost  
Means of Transportation: Transport Van 
Leave school: 10:40 am
Arrive back at school: 1:20 pm

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Engineers of the Future during the field trip. In exchange for the acceptance of said child’s candidacy by Engineers of the Future. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Engineers of the Future . and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said student, I hereby waive all claims against  Engineers of the Future including all Mentors and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. 

Media Release and Authorization Release

As the Parent and/or Guardian of the named student, I hereby grant permission for the use of photographs, videos, and other media depicting the named scholar for promotional and educational purposes by Engineers of the Future and its affiliates, including Mentors, clients, sponsors and media.

I understand that these materials may be used in print publications, online platforms, social media channels, and other promotional materials associated with Engineers of the Future, without further consent or compensation.

This release is authorized and executed of my own free will, with the sole purpose of allowing the use of media materials for promotional and educational purposes related to the activities of Engineers of the Future.

 

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
