I hereby acknowledge and confirm that I am the legal parent/guardian of the namesake collection nominee. I give my full and unreserved permission to Laree + Co. to use my child's name, story, photos, and so on in association with one of their product collections.

I understand and agree that this permission is granted freely and without any expectation of compensation or remuneration. I acknowledge that the use of my child's name in connection with Laree + Co.'s collection does not entitle me or my child to any form of payment or profit sharing.

Furthermore, I release Laree + Co., its affiliates, successors, and assigns from any and all liabilities, claims, demands, or actions that may arise from or in connection with the use of my child's name in their product collections.

I confirm that I have read, fully understand, and agree to the terms and conditions set forth in this acknowledgment and permission form. This agreement is binding upon me, my heirs, executors, legal representatives, and assigns.
