  • I certify that neither I, nor anyone living in my household, has ever been charged with animal cruelty, neglect, or abandonment.*

  • I voluntarily enter into this agreement to provide a temporary home as a foster caregiver to any cats/kittens Witches Kittens may temporarily place in my care. I understand and agree not to relinquish custody of any foster cats to anyone without receiving prior approval from Witches Kittens. I understand that Witches Kittens provides no guarantee as to the health of my foster cat and that my foster cat might have medical needs, socialization/behavioral problems, and may not be litter box trained. I agree to immediatly let Witches Kittens know of any medical issues that arrise while the cat(s) are in my care and Witches Kittens will arrange for and  provide veterinary care as they deem necessary. I agree that I am fostering this animal and that I have no right of ownership over my foster animal. I agree to release Witches Kittens from all responsibilities that may occur while a foster pet is in my care. I agree to provide a warm and loving environment to my foster animal.

    By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully. I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any animal fostered by me. I certify that the above information is correct, and I understand that the information will be verified. I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Witches Kittens Inc and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster animals. I accept full responsibility for the cat(s) actions at all times, and release Witches Kittens Inc from any liabilities or damages that may be incurred because of fostering such cat(s). I agree to have Witches Kittens complete reference call checks and conduct a home visit inspection to be able to approve my foster application. I agree that if I'm unable to foster the cats(s) anymore that I will return the cat(s) to Witches Kittens and try to give Witches Kittens a 2 week period to try and find another suitable foster for the cat(s).
