2025 Black Hawk College Alumni Hall of Fame Nomination Form

Do you know a remarkable Black Hawk College graduate who deserves recognition for extraordinary professional, national, or civic achievements?

The Board of Directors of Black Hawk College Foundation is now accepting nominations for outstanding alumni to be considered for induction into the 2025 Alumni Hall of Fame. The Foundation’s Alumni Hall of Fame recognizes alumni for their outstanding achievements in their chosen field and/or their community. Nominations will be evaluated based on the following criteria:

• Outstanding distinction and success in chosen field
• Humanitarian service which has proven beneficial to society
• Completed at least 30 college credit hours at Black Hawk College

Please help us by identify deserving candidates for this noteworthy award. If you know a Black Hawk College alumnus worthy of recognition who meets the criteria, please complete the nomination form, and return it to the Black Hawk College Foundation Office by 5 p.m. on January 15, 2025.

As a nominator you will be invited for the induction ceremony to be held on Thursday, April 3, 2025, during our Annual Black & Gold Celebration event.
