Self-certification of fitness

I, acknowledge, agree, and represent that I understand the nature of this Kids Against Hunger Canada Meal Pack and that I am qualified, in good health, and in proper physical condition to participate in such activity. I have not recently tested positive to Covid-19 (Coronavirus and its variants) - and if so, I have followed the appropriate quarantine period since catching the Covid-19 virus (or variant virus strains I am not experiencing any of the following health conditions: severe difficulty breathing, or severe chest pain, or confusion, or loss of consciousness.

Assumption of Risk and Indemnity

I further agree and warrant that both parties to this agreement jointly hold each other harmless and waive all rights of subrogation regarding any and all liability for bodily injury and property damage that may occur during and connected to this Kids Against Hunger Canada meal pack event.

Agreements to Food Packing

*I will not bring any food or drinks into the area designated for the Meal Pack.

*I will wear a hairnet and place it on my head before washing my hands.

*I will remove all jewelry, other than a wedding ring or medical information jewelry, before washing hands.

*I will wash my hands with either soap and water and anti-microbial wipes, and/or use hand sanitizers.

*I will wear food handling gloves (provided) throughout the event.

*I will observe safety measures and avoid touching my face and cell phone.

*I will not handle or use my cell phone on the food packing line. If the use of my phone is needed, *I will step away from the packing line for its use, then rewash, and put on new gloves afterwards.

*I will wear a KAHC apron in the food pack area, and will remove the apron before entering a washroom.

NOTE: The Use of FACE MASKS are completely OPTIONAL to each individual.

Photo/Video Release

I, hereby consent to being photographed, filmed or videotaped by representatives or agents of Kids Against Hunger Canada, and I hereby consent to and authorize the use of photos by Kids Against Hunger Canada or any individual or entity authorized by Kids Against Hunger Canada, of any and all photographs, film and videotapes taken of me. I further understand that the negatives, photographs and videos shall be the property of Kids Against Hunger Canada, and that they shall own the copyright to all such materials.

Minor Release

And I, the Minor's PARENT and/or LEGAL GUARDIAN, understand the nature of Kids Against Hunger Meal Pack Events and the Minor's experience and capabilities, and believe the Minor to be qualified, in good health, and in proper physical condition to participate in such activity.
