Please complete this form to register your Dog or Cat. Pursuant to City Code, Chapter 90.007
Annual (yearly) registration is required for all dogs and cats over 3 months of age residing in the City of Tool. A separate form is required for each animal. No more than four (4) dogs or four (4) cats or in any combination of such is allowed per residence. Registration expires twelve (12) months after the date of most recent rabies vaccination.

Successful registration requires the following:

1. Completed form per animal

2. Proof of current rabies vaccination

3. Photo of your animal

4. Registration fee - $10 per animal

5. Reading Chapter 90.007 of the City of Tool's Code Ordinance
