BFS Coral Springs Soccer Registration

Please complete all required information below. If adding more than one child to the pre-registration, please fill in all information for each child. If you need to add more than two children, please complete additional forms as needed or email us at BFSCoralSprings@gmail.com to add additional children.

Session Dates: 

Mondays and Wednesdays starting April 1. Fee is for one month.

All clinics will be held at North Community Park North Field. 

Preliminary session times based on birth year:

     2015-2018 5:00 pm

     2011-2014 6:00 pm

We will evaluate all players and determine if the groups need to be redistributed based on age and skill. We will not move your child's time without discussing with you and your agreement. 

Cost is $150 a month for the first child and $130 for each additional child. 

Drop-In cost $25 a session, discounts may be available if registering for multiple sessions. Availability depending on capacity. 

All sessions will be held at North Coral Springs and surrounding fields. Session times will be 5 or 6pm depending on age and skill. If your player has not yet been evaluated, we will assign a preliminary session time based on age and advise if any adjustments are needed. 

Discounts available for referrals, active miliatry, and groups. Please add a special note.

Weather: all sessions will be held as scheduled unless the filed is closed (e.g. lightning alarm). We do our best to make up any weather related cancellations, however make ups are dependent on field availability and are not guaranteed. 

Behavior: all parents and players are expected to behave in a respectful and safe manner at all times. We reserve the right to remove any player from the program for the safety and wellbeing of the other players.

No refunds. 

By completing this registration you agree and acknowlege that your child is healthy and does not have any medical concerns that would prohibit them from safely participating in the activity. 

FLORIDA MINOR RELEASE AND WAIVER OF LEGAL LIABILITY

THIS IS YOUR RELEASE AND WAIVER OF LIABILITY (the “Release”). You individually and on behalf of your minor child(ren), release BFS Coral Springs, its managers, officers, owners, employees, volunteers, agents, independent contractors, otherparticipants and/or others acting on its behalf (collectively, “BFS”). You agree that this Release is effective immediately.

NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN  READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF BFS USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM BFS IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND BFS HAS THE RIGHT TO REFUSE TO LET YOUR CHILD
PARTICIPATE IF YOU DO NOT SIGN THIS FORM.
