By signing this form, I hereby give permission to Planet Ark, it's sponsors and supporters, for their use of any photographs taken of me (and/or my children) for any purpose promoting National Tree Day or Schools Tree Day.

Safety

The participant agrees that Planet Ark Environmental Foundation, Toyota and/or any Event/Site Coordinators participating in National Tree Day will be neither liable nor in any way legally responsible for any:
1. Injuries sustained by the participant;
2. Loss or damage to property owned by or in the possession of the participant; or
3. Act or omissions, negligence or fault of any person participating in National Tree Day.
Insurance

Planet Ark has arranged death, injury and disability insurance cover for participants in registered public National Tree Day activities. For this cover to be effective, each participant (including event/site coordinators) must complete the 'Participation Registration Form'.

Note: Children under the age of 18 must have a parent or guardian sign for them. All family members must complete the form.
