    St. John Lutheran Church and School requires that ALL volunteers (18 years and older) working with children or youth in service to our ministry fill out and submit a Personal Lifestyle Screening Form annually. 1. No individual may volunteer at SJL in a role involving potentially unsupervised contact with children or youth if they have been convicted of a crime involving sexual abuse, physical abuse, morally corrupt behavior, or have a felony criminal record. 2. Persons who have been convicted of DWI/DUI, a crime involving controlled substances, or who have demonstrated a pattern of alcohol or substance abuse, must be clean and sober for a minimum of two years before working with children or youth. 3. Volunteers must immediately report suspected or observed abusive behavior to a pastor, teacher, or the principal.
  • St. John Lutheran may request a background check on you. The background check may require the submission of your fingerprints. Refusal to submit to a background check terminates the volunteer process. You have a right to obtain a copy of the report resulting from the background check. You also have the right to challenge its accuracy. Pending completion of the background check, your access to children and youth may be limited, delayed, or denied.


    I give permission for St. John Lutheran to obtain information relating to my criminal history record, including arrest and conviction data as well as plea bargains and deferred adjudication. I understand that this information, including a formal, written background check, will be used, in part, to determine my eligibility for volunteering within the SJL community.

    I understand my rights and responsibilities as outline above. I certify that all of the information that I provided on this form is accurate. I understand that providing incorrect information is cause for immediate dismissal.

    If approved to volunteer, I agree to be bound by the bylaws and policies of St. John Lutheran Church and School and to perform my services on behalf of the church and school in accordance with Biblical standards of conduct.

    This is a legally binding agreement, which I have read and understand.
