CLASS START & END TIMES
All students are expected to arrive on time & all minor students must be picked up promptly. Classes start & end at the advertised times. If a student arrives late, they will not be given extra time at the end of class to finish their project. If a minor is not picked up on time extra may charges apply.
DROP OFF + PICK UP PROTOCOL
Space is limited, however parents and guarians are welcome to stay. Please note that depending on weather and heat, classes will be held inside our home or in the garage. Once we find a more permanent home for the studio all classes will move to the new location, as we are currently looking for studio space. Children can only be picked up by parents or guardians. If anyone else will be picking up, we ask that you notify Gogh Create Studio via email before the start of class.
PHOTO RELEASE
Gogh Create Studio may take photographs or videos of artwork or work in progress at the studio during classes which may/ or may not be used for publicity purposes. These photos may be published on our social media pages or platforms &/or website, &/or may be posted on other promotional materials or in other promotional media. NO STUDENT will be identified in these photographs or tagged/identified in any photo without their consent. Please note, we archive all work created at our studio. If you do not wish for your child to be in a group photo, please request to opt out via email at kmcnaughton@goghcreatestudio.com . All photographs are the property of Gogh Create Studio and we do not allow these photos to be published by third-party or by any other media or source. We value & are protective of our students, their interests & privacy.
SNACKS, DRINKS, BELONGINGS
Please do not share any belongings, food, or drinks with any other student or staff member. Please do not bring any non-essential items to our studio. This includes bags, bicycles, toys, stuffed animals. Water bottles are permitted. Gogh Create Studio is not responsible for lost or stolen belongings.
Legal/ Liability:
I CERTIFY that I am the parent or legal guardian of the above-mentioned minor participant(s) mentioned herein & confirm that the information I have entered here is accurate and true. I CERTIFY that I am responsible for any caregiver or guardian mentioned herein and guarantee that all of the above is true and correct. I CERTIFY that I am at least 18 years old and I have read and agree to the terms of the above agreement.
GOGH CREATE STUDIO LLC
THIS DOCUMENT RELEASES AND WAIVES CERTAIN LEGAL RIGHTS. YOU SHOULD READ THIS DOCUMENT THOROUGHLY AND YOUR SIGNATURE (AS DENOTED BY YOUR ELECTRONIC REGISTRATION FOR THIS CLASS OR SIGNATURE ON A HARD-COPY FORM AT THE STUDIO) REPRESENTS THAT YOU HAVE READ AND UNDERSTAND THIS AGREEMENT. THIS APPLIES TO ALL PARTICIPANTS UNDER A GROUP SIGN-UP, IF THAT IS HOW PARTIES WERE REGISTERED.
On behalf of myself, my personal representatives, heirs, next-of-kin, spouse and assigns, I acknowledge that I hereby give up certain legal rights when I register for a class or event which includes the right to recover damages to personal property, bodily injury, and any projects left at the studio in exchange for the consideration of participating in the class and using the tools and equipment provided to me as part of the class.
Furthermore, I HEREBY:
Release Gogh Create Studio LLC from any loss, liability, damage, injury or cost that may arise out of my use of art materials, tools or equipment at Gogh Create Studio LLC and I voluntarily assume the risk and danger of injury using such art materials, tools and equipment.
Release, discharge and promise not to sue Gogh Create Studio LLC under its own name or any other name and/or any of its owners, officers, employees, instructors, and agents (hereinafter, “Releasees”) for any loss, liability, damage, injury, illness* or cost whatsoever arising out of or related to my person or property.
Agree to abide by and follow instructions and rules established by Gogh Create Studio LLC, or any of its instructors, agents, or employees, with regard to safety operations in using working tools or other artistic equipment. This includes slippery surfaces from paint clean-up, etc.
Acknowledge that this document is a contract and agree that if a lawsuit is filed against Gogh Create Studio LLC or its owners, agents, employees or instructors for any injury or damage in breach of this contract, the undersigned will pay all attorney fees and costs incurred by Gogh Create Studio LLC in defending such action. The registrant or guardian expressly agrees that the foregoing release and waiver of liability, assumption of risk, and indemnity is governed by the State of Arizona and intended to be as broad and inclusive as is permitted by Arizona law, and that in the event any portion of this Agreement is determined to be invalid, illegal or unenforceable, the validity, legality and enforceability of the balance of the agreement shall not be affected or impaired in any way and shall continue in full legal force and effect.
Understand that when I provide my email address at registration, I may be added to the email list for periodic emails from Gogh Create Studio LLC, but that I will have the option to unsubscribe at the bottom of the emails.
By registering for the class or event I acknowledge that I have read this document in its entirety and agree to the conditions.
