Consent, Policy, and Waiver

21 Day Transformation Challenge

Participants Agreement and Release Form

I hereby acknowledge that I have voluntarily enrolled in the 21 Day Transformation Challenge organized by Project Blossom District, to be held in the state of Florida.

1. Entry Fee: I understand that there is a non-refundable entry fee of $5 required to participate in the Challenge. I agree to pay this fee upon signing up for the Challenge.
2. Before and After Pictures: I understand that as part of the Challenge, I am required to submit before and after pictures of my transformation. These pictures will be used for judging purposes and may be used by Project Blossom District for promotional purposes.
3. Judges Policy: I acknowledge and agree that the judging of the transformation pictures will be conducted by impartial judges appointed by Project Blossom District. The judges’ decisions are final and binding.
4. Submission Deadline: I understand that the before and after pictures must be submitted no later than [May 28th, 2024]. Failure to submit pictures by this deadline may result in disqualification from the Challenge.
5. Consent to Use Images: I hereby grant Project Blossom District the irrevocable and unrestricted right to use, publish, and display my before and after pictures, as well as any other images, posts, or likeness related to my participation in the Challenge, for promotional purposes. This includes but is not limited to the promotion of the current Challenge, future challenges, and any related content.
6. Release of Liability: I understand and acknowledge the risks associated with participating in physical fitness activities. I release Project Blossom District, its officers, employees, agents, and affiliates from any and all liability, claims, damages, or injuries arising out of or in connection with my participation in the Challenge.
7. Compliance with Rules: I agree to abide by all rules and regulations set forth by Project Blossom District for the Challenge. I understand that failure to comply with these rules may result in disqualification from the Challenge.

I have read and understood the terms and conditions outlined in this Consent, Policy, and Waiver, and I voluntarily agree to participate in the 21 Day Transformation Challenge under these terms.
