    PROMOTION REGISTRATION INFORMATION

    As the parent of a 6th grader, your help is needed to ensure the Class of 2024 has a special closing to their years at Silver Oak. We hope your child will participate and celebrate along with their Silver Oak classmates by registering to attend all events.

    All activities, except the Promotion Ceremony, are optional. Registration is required to attend the dance and pool party as walk-ins are not allowed. The Aloha Skating Rink 6th Grade Trip is organized by Silver Oak, so details will continue to be communicated directly by the school.

    Please complete this form by selecting which optional activities you plan to have your 6th grader attend, and submit payment in one of the following ways:

    PAYMENT OPTION 1 - Cash or Check payable to our Promotion Treasurer, ANGELINA LU. Include the Student's Name and Teacher in the memo. Payments may be dropped off in the 6th Grade Promotions envelope in the PTO mailbox in the office, or submitted to your child's teacher.

    PAYMENT OPTION 2 - Via mobile payment app - Include the following info: Student's Name and Teacher

    Please make sure to register in advance. We will not be able to accept payment or permission slips at the events. Furthermore, we cannot issue refunds if your child changes their mind about attending an event. However, if an event must be cancelled, all registrants will have the opportunity to have their fee(s) refunded. If you have more than one child in the 6th grade, please complete a separate form for each.

  • COMMEMORATIVE ITEMS & CLASS GIFT

    Each 6th grader will receive an embroidered class blanket and participate in a signature tile design at no cost to the family. Each family has the option to order a personalized Class of 2024 Lawn Sign, and the opportunity to contribute toward the gift that the Class of 2024 will give to Silver Oak. The Promotion Committee is actively working to identify something special that the 6th graders can gift to their alma mater, Silver Oak. Your donation helps make these traditions possible, and special.
  • CLASS OF 2024 SIGNATURE TILE
          *INCLUDED - NO CHARGE*
    All 6th Graders will have the opportunity to autograph the Class Signature Tile. This tile will be personalized by the Class of 2024, fired in the kiln, and mounted on the exterior school wall at Silver Oak.

    All 6th Graders will receive an embroidered Class of 2024 blanket to remember their Silver Oak Promotion Class!

  • POOL PARTY RELEASE OF LIABILITY AGREEMENT

    I agree to release, hold harmless and indemnify the 2022 Promotion Committee members and party chaperones from and against any claim for losses, damages or injuries resulting from any cause in connection with participation in the 6th Grade Pool Party. I am aware that a pool environment includes certain risks and dangers, and I voluntarily accept responsibility on behalf of my child for all risks involved in attending a pool event. I further understand that this is a privately organized event, and Silver Oak Elementary, nor Evergreen Elementary School District have any responsibility or liability for this event.

  • POOL PARTY RULES

    • Only Silver Oak 6th Grade students may participate.
    • This event is privately arranged and is not sponsored by the school or district.
    • Student will wear appropriate swim attire (no denim, cut-offs or shorts in the pool)
    • Student will abide by the Creekside Cabana swimming pool rules of behavior - no running, no hazardous activities, no pushing, dunking or dangerous games.
    • Student will behave in a courteous, respectful, and safe manner in compliance with Silver Oak Standards of Behavior.
    • A lifeguard will be on duty, but children who do not swim are asked to enjoy the festivities poolside.
    • If a student violates any of the rules, his/her parent(s) will be notified and the student will be sent home. If the student is sent home, a parent or parent designee must pick up student within 20 minutes of notification.
    • Once a participating student has checked in, he/she may not leave the pool until the parent or parent designee signs the student out (unless permission has been granted to allow the student to walk).
    • No in/out priviledges during the pool party.
    • No admittance to the adult side of the pool property.
    • Event ends at 4 p.m. Your child must be picked up from the Creekside Cabana by 4p.m.
  • 6th GRADE DANCE RULES

    • Only Silver Oak 6th Grade students may participate.
    • Students will be checked in and dropped off at the door by the flagpole - no parent admittance (adult chaperones will be present throughout the dance).
    • DRESS CODE: NO strapless dresses, tube tops, or bandeau tops. Dresses and shorts must be finger tip length or longer. Dress sandals are permitted - NO flip-flops. Soft shorts are recommended under dresses.
    • Student will behave in a courteous, respectful, and safe manner in compliance with Silver Oak Standards of Behavior.
    • If a student violates any of the rules, his/her parent(s) will be notified and the student will be sent home. If the stduent is sent home, a parent or parent designee will pick up student within 20 minutes of notification.
    • No in/out priviledges during the dance. Once a participating student has checked in, he/she may not leave the dance until the parent or parent designee signs the student out.
    • Event ends at 7 p.m. Your child must be picked up promptly at 7 p.m.
