Agreements
My student understands and agrees to give the best care to any rented instrument and all school owned equipment. Any need for repair or adjustment will be reported to the instructor. We understand all repairs must be made by a BPS employed professional.

My student agrees to abide by rules set by the instructor for use, storage and transportation of any school owned instrument. Any instrument will be stored it in its proper case or on an approved stand, and no instrument will be left in a vehicle or in conditions of extreme heat or cold.

I, the Parent/Guardian, understand that it is my responsibility to pay for any and all damage to my student’s school owned instrument, including percussion.

An optional accidental damage waiver is available for purchase. The waiver must be purchased at the time of rental and cannot be purchased at a later date. The waiver is non-refundable and will cover the cost of instrument repairs necessary due to accidental damage. It does not cover consumable items (reeds, strings, etc.). It does not cover damage due to student negligence.

The waiver is valid only for the school year. A summer waiver is not available.
I understand that I am responsible for all instrument damage. I understand I have the option to purchase a waiver to cover accidental damage.*

I understand that this agreement is in effect for a maximum of one school year, and that this agreement will terminate immediately if my student drops out of music. I further understand that no refund will be issued in the case of withdrawl.
I have read and understand the effective dates of the rental agreement.*

I understand that all instruments and issued accessories will be returned to the music instructor at the end of the school year, or the end of the summer in the case of a summer rental, or immediately upon withdrawal from the music program.
All instruments and accessories will be promptly returned at the end of the contract period, or when my student withdraws from music, whichever is sooner.*
