Acknowledgment of Risks:
I, the Parent/Guardian, understand that my child/ward’s participation in Refuge Youth’s overnight trip to The Gathering 2024 youth conference organized by NextGenMB involves certain risks and hazards, and I acknowledge that I am voluntarily allowing my child/ward to participate in this trip. I agree that my child/ward will abide by all safety instructions provided by the trip leaders and exercise caution and good judgment at all times during the trip.
Release of Liability:
In consideration of being permitted to participate in the overnight trip, I hereby release and discharge both NextGenMB and Refuge Youth, their officers, directors, employees, volunteers, and agents from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may occur during the trip, including but not limited to claims for negligence, breach of contract, or breach of statutory duty.
Medical Information:
I certify that my child/ward is in good health and physical condition to participate in the overnight trip. I understand that it is my responsibility to inform the trip leaders of any medical conditions, allergies, or medications that may affect my child/ward’s participation in the trip.
In the event of a medical emergency, I authorize the trip leaders and/or designated medical personnel to administer emergency medical treatment, including but not limited to first aid, CPR, and transportation to a medical facility.
