Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Global Reach Inc during the selected camp. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Global Reach Inc and all its respective officers, teachers, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Global Reach Inc., including all teachers and affiliates, all participants, sponsors, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities.

Medical Release and Authorization

As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is also granted to the Global Reach Inc and its affiliates including officers and teachers to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Cancellation Policy and Refund

Requests for cancellation and refunds at 100% must be submitted in writing to globalreachcolorado@gmail.com at least fourteen (14) days in advance of the camp start date for consideration.

Refunds for cancellations occurring less than fourteen (14) days in advance are only considered in cases of emergency and handled on a case-by-case basis. The cancellation will be subject to $100 fee.

Refunds are NOT issued for early departures from camp, campers who choose not to stay at camp during drop off, campers considered “no shows” (those who do not attend and did not give advance notification of cancellation), or for days missed during a camp session.

This refund policy will be strictly enforced for all camp registrants.

Camp Fee

Total fee for the camp, including housing, meals, and activities:

1st Session: $555.00

If registered before May 20th: $510.00.
2nd sibling discount: 5% ($527.75 after May 14th or $484.50 before May 14th)

2nd Session: $630.00

If registered before June 7th: $585.00.
2nd sibling discount: 5% ($598.50 after June 7th or $555.75 before June 7th)

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
