Dermaplaning Consent

Dermaplaning is a physical/mechanical form of exfoliation using a specialized dermaplaning blade for the removal of built up dead skin cells and vellous hair. Following treatment skin will be smoother, softer and better able to absorb the active ingredients in treatment and home care products.

Please read and initial the following:

I understand there are contraindications to this treatment, including but not limited to, diabetes (not controlled by diet or medication), cancer, active acne, bleeding disorders, the inability for blood to coagulate or the development of keloids following injury. Certain medications including blood thinners, higher dosages of Aspirin, and Accutane are contraindicated for this treatment due to the possibility of delayed clotting from a nick or cut. I certify that I am not taking any of the above medications or experiencing any of the above conditions.

Dermaplaning will gently exfoliate and eliminate surface layers of dead skin cells and unwanted vellus hair (peachfuzz) and will prime skin for our professional enzyme or acid peel: included, a Professional Enzyme or acid peel (immediately unveils smoother, more vibrant, and younger-looking skin) Formulated with an exclusive blend of clinical-grade enzymes and/or acids. Professional enzymes or acid peels dramatically diminishes the appearance of discoloration, while smoothing texture and renewing the appearance of pores, to restore a brighter, more clarified skin tone. For optimal results, a series of 4-6 treatments every 4 weeks is recommended.

RISKS: All things considered, Dermaplaning is very safe. Of course bleeding is possible, the treatment involves the use of a sharp surgical blade. (and haven’t you ever cut yourself shaving before?). As with your own shaving experiences in the shower, cuts are extremely rare and more annoying than painful. Makeup may be applied after Dermaplaning, in fact you may note that your makeup glides on and sits much better on your face after a session than before.

Skin Facial Bar, LLC will take every precaution to ensure that your skin is well hydrated and calm following each session.

However, you may experience excessive dryness or even some peeling between sessions, which may or may not be normal. Always contact Skin Facial Bar, LLC if you have any concerns. More sensitive skin may experience some redness after the first couple of sessions. This normally goes away after 2 to 3 hours. Dermaplaning may cause minor superficial abrasions which may not appear until a day or two following your treatment. If this should occur, please contact me so that I can do a post-treatment follow up with you.

After your treatment, SPF 30+ MUST be worn at all times.

RESULTS: The majority of Skin Facial Bar, LLC clients receive noticeable, satisfactory, above average results with a series of treatments and a commitment to a daily skin care regimen. However, this outcome cannot be guaranteed as maximum results are highly dependent on age, cumulative sun exposure, health, lifestyle, genetic traits, general skin condition, and willingness to follow recommended protocols. Be aware that many changes may occur deeper within the skin over time. To continue the maintenance of your skin after you complete your treatment(s), Skin Facial Bar, LLC may inform you of other preventive treatments they may benefit your skin help with management of your skin.

PRE TREATMENT INSTRUCTIONS

Please discontinue products containing any exfoliating agents (retinoic acid, tretinoin, retinol, benzoyl peroxide, glycolic acid, salicylic acid, astringents, etc.) at least 72 hours before treatment.
A history of herpes or cold sores may require a course of anti-viral medication pre and post treatment.
Your provider will recommend appropriate products to help enhance results and/or minimize potential side effects.
Isotretinoin (Anti-Acne), also know as Accutane. You can be treated if the medication has not been taken within the last six months.

POST MAINTENANCE INSTRUCTIONS

You may experience slight redness and swelling, which should resolve within 24 hours.
Apply SPF 30 and UV Ray protection daily post treatment.
Mineral make-up can be worn immediately following treatment.
Avoid heat, saunas, hot tubs and sweaty activity for 24 hours; this includes exercise of any kind.
Avoid products containing any exfoliating agents (retinoic acid, retinol, tretinoin, retinol, benzoyl peroxide, glycolic acid, salicylic acid, astringents, etc.) for 4-7 days post treatment.
You may resume your regular skin care routine 24 hours after treatment.

POST-TREATMENT/HOME CARE: Exercise or vigorous physical activity should be avoided until all redness has subsided. Direct sunlight exposure is to be completely avoided immediately following the treatment (including any strong UV light exposure or tanning beds). Although SPF 30+ should already be a part of your daily skin care, after dermaplaning, SPF 30+ must be applied daily to the treated area for a minimum of two weeks. Twice daily cleanse the treated area with a post treatment cleanser, followed by a serum or treatment cream and follow with SPF 30+ sunscreen.

I am 18 years of age or older and have informed the skin care specialist of any physical or psychiatric health problems that would prevent me from having this procedure performed, and I know of no reason why I should not have these procedures performed on me. I understand that temporary redness, swelling, bruising and discomfort occur from this procedure. Possible complications that could occur include, but are not limited to, risk of infection, allergy or sensitivity to local anesthetics and inconsistent results. I will also seek medical attention as recommended by skin care specialist if necessary and understand that I am responsible for the full payment of expenses incurred in the event this is necessary.

This procedure is being performed under standard sanitizing and sterilizing methods as recommended by the Centers for Disease Control and as required by the State Department of Health. All needles used of are disposed of properly after each procedure.

In consideration of the skin care specialist providing me with the service requested, I for myself, my spouse, legal representatives, heirs, and assigns, hereby release, waive and discharge the skin care specialist along with Serenity Day Spa & Salon, Inc. from liability for all loss of damage on account of or injury to person. I understand several procedures are necessary to achieve the desired effect and agree to complete my treatments as recommended. Should I not complete treatments, I will be responsible for any adverse outcome.

I expressly agree that this consent, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws in the State of Georgia. I have read this consent and understand all its terms and execute this release voluntarily, and with full knowledge of its significance. All my questions have been answered satisfactorily prior to signing of this consent.
