When: Thursday, July 3, 2025 - 5:00 PM until Completion 

Where: Coon Rapids Ice Center/Blvd. Plaza

Who: 2 person teams / $60.00 per team entry fee - MAX of 24 Teams

Check in will begin at 4:00 PM

Tournament will start at 5:00 PM SHARP

ANY TEAM NOT CHECKED IN BY 4:45 AM WILL BE REMOVED FROM THE EVENT

Payouts:

1st Place = $150 to the team

2nd Place = $100 to the team

3rd Place = $75 to the team

Waiver:

By signing this form, I/we verify the following:

1. By signing this form, both players affirm that all the information herein is true and correct to the best of their knowledge. Any false information or misrepresentation that shall be found may cause the disqualification of such individual and penalty may be incurred against the team.

2. Each player on the team agrees to abide by the rules and regulations as set forth by the 4th of July Committee. Tournament directors will have ultimate decision making authority on any matters that might arise concerning any one of the teams and/or players. Failure to abide by any rules and regulations can jeopardize the team's participation in the tournament.

3. We, as team participants, hereby agree to allow the players named herein to participate in the aforementioned tournament and I further agree to indemnify and hold the 4th of July Committee, City of Coon Rapids, and the Coon Rapids Police Association harmless from and against any and all liability for any injury which may be suffered by the aforementioned player(s) rising out of, or in any way connected with, his/her participation in this tournament.

4. If unable to participate positively, I/we understand that I/we may be removed from the tournament immediately and future participation will be pending.
