Thank you for choosing Lee’s Rascal Retreat for your pet boarding needs. To ensure the best care for your pet and to maintain a safe and efficient environment, we require all pet owners to agree to the following terms and conditions before utilizing our services:
Ownership Confirmation:
I confirm that I am the legal owner of the pet, or have been authorized by the legal owner to make decisions regarding the pet's care.
Health Assurance:
I assure Lee’s Rascal Retreat that my pet is currently in good health and has been examined by a veterinarian within the last year.
Vaccination Confirmation:
I confirm that my pet is up-to-date with all vaccinations required by local laws and regulations, and I will provide documentation verifying this upon request.
Safety and Comfort:
I acknowledge the requirement for my pet to wear a safety collar while at Lee’s Rascal Retreat, and I will provide one if my pet does not already have it.
Parasite and Disease Certification:
I confirm that my pet is free from fleas, ticks, and any communicable diseases, as certified by a recent veterinary exam (within the last 30 days).
Liability Waiver:
I release Lee’s Rascal Retreat and its employees from liability for any harm that may come to my pet, including but not limited to injury, sickness, damage, accident, or death, provided that such harm is not the result of gross negligence or intentional misconduct by Lee’s Rascal Retreat or its employees.
Delayed Pickup Policy:
In the event that I am unable to pick up my pet at the agreed-upon time, I authorize Lee’s Rascal Retreat to contact me to arrange for continued care, which may incur additional fees.
Emergency Medical Care:
I authorize Lee’s Rascal Retreat to provide my pet with emergency medical attention from an in-house or local veterinarian if deemed necessary by Lee’s Rascal Retreat.
Holiday Reservations:
For boarding during holiday periods, reservations must be made at least one week in advance. Availability during these times is limited and subject to increased rates.
Accuracy of Information:
I confirm that all information provided in this registration form is true and accurate. I agree to update Lee’s Rascal Retreat with any changes to this information.
Cancellations and Refunds:
Cancellations must be made at least 48 hours prior to the scheduled boarding date to be eligible for a refund. Cancellations made less than 48 hours before the scheduled date may incur a fee.
Personal Items Policy:
While personal items such as bedding and toys may be brought to Lee’s Rascal Retreat, I acknowledge that the safety of these items cannot be guaranteed and Lee’s Rascal Retreat will not be held liable for any damage or loss.
Behavioral Agreement:
I agree that my pet does not have a history of aggression and is socially compatible with other animals and people. Lee’s Rascal Retreat reserves the right to refuse or terminate service if my pet exhibits aggressive or unsafe behavior.
