JOSHITAI FITNESS, LLC works on a paid Loyalty Fitness card basis. There will be a Loyalty Fitness card presented to members/attendees on their first fitness visit. The Loyalty Fitness card represents each attendance and payment for each fitness class completed; the card will be stamped accordingly by the Fitness Instructor stamp only.

All cardio fitness classes are $10.00 with some exceptions according to HEC Fitness Ministry financial and/or status  criteria.

Bootcamp and/or Personal Training prices will vary in different personal circumstances according to HEC Fitness Ministry financial and/or status criteria.

Payments will be made via CashApp, Venmo, PayPal or cash only.

Fitness Training Waiver and Release of Liability
I, the undersigned, hereby acknowledge and desires to use the fitness facility and equipment at 702 Atascocita Road, Humble, TX 77396 by Joshitai Fitness LLC, in consideration of the opportunity to do so, Member agrees as follows:

Member understands that using the fitness facility and equipment involves certain inherent risks, including but not limited to the following specific risks:
1. Use of equipment: Risks associated with using gym equipment such as weights,machines, and cardiovascular equipment.
2. Physical activity: Risks associated with participating in physical activities such as classes, training sessions, or personal coaching.
3. Slip and fall accidents: Risks associated with slipping, tripping, or falling while in the gym, including those resulting from spills, wet surfaces, or uneven flooring.
4. Strains and sprains: Risks associated with strains, sprains, and overuse injuries from participating in physical activity or using gym equipment.
5. Cardiovascular conditions: Risks associated with engaging in physical activity,
  including heart attack, stroke, and other cardiovascular events.
6. Musculoskeletal injuries: Risks associated with musculoskeletal injuries such as torn muscles, ligaments, or tendons.
7. Heat stroke: Risks associated with overheating, dehydration, or heat stroke resulting from participating in physical activity.
8. Allergic reactions: Risks associated with allergies or adverse reactions to equipment, products, or substances used in the gym.
Assumption of Risks:
Member, for himself/herself and for his/her executors, administrators, and assigns, knowingly and voluntarily assumes all risks associated with using the fitness facility and equipment, including those risks listed above. Members acknowledge all risks associated with Hip Hop Stepping, Tabata, and (HIIT) training can have potential risks of intense cardiovascular exerciseand the importance of proper hydration.

Waiver and Release:
Member, releases, waives, discharges, and covenants not to sue the facility, its owners, employees, agents, and contractors from any and all liability, claims, demands, causes of action, damages, or expenses arising from any injury, harm, or death, howsoever caused, to Member, whether arising from the negligence of the facility, its owners, employees, agents, or contractors, or otherwise.

Medical Representation:
I represent that I am physically fit to use the facility and participate in physical exercise. I have no medical condition that would prevent my safe participation. If I have any medical conditions or concerns, I have consulted with a healthcare provider and obtained clearance to participate.

Member acknowledges that he/she has read, understood, and had an opportunity to ask questions about the Waiver and that he/she has been given sufficient time to review the Waiver before signing.

This Waiver and Release of Liability shall be binding upon and inure to the benefit of the parties,their respective heirs, executors, administrators, and assigns.

Rules and Regulations:
I agree to abide by all rules, regulations, and policies of the Provider, including proper use ofequipment, following safety guidelines, and respecting other members and staff.

Consent to Medical Treatment:
I hereby consent to receive any necessary medical treatment resulting from my use of the gymfacilities and agree to bear all costs associated with such treatment.

Acknowledgment:
I have read this Fitness Class Waiver and Release Agreement, understand its contents, and agree to be bound by its terms. I understand that I am giving up substantial legal rights by signing this document.
