I authorize my child to participate in the Dennis Gardeck and Mikeys League Inclusive Summer Kickoff [event], on the date listed below (the “Event”). I understand that there are risks and dangers involved in the Event (including risks that are not known or not reasonably foreseeable), and I voluntarily assume all those risks and dangers, including the risks and dangers of personal injury and property damage that may occur during or arise from the Event. In exchange for being permitted to participate in the Event, I hereby expressly and unequivocally release and discharge Arizona Cardinals Football Club LLC, the National Football League, Mikeys League, and the Dennis Gardeck Foundation, its thirty-two (32) member professional football teams (including the Arizona Cardinals), NFL Properties LLC, NFL Ventures L.P., the sponsor(s) (if any) of the Event, and each of their respective officers, directors, members, managers, employees, volunteers, subcontractors, sponsors, agents, affiliates, subsidiaries and shareholders (collectively the “Releases”) from all present and future claims that may be made by me, my family, estate, heirs or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the Event wherever, whenever or however the same may occur. I further release, waive, discharge and covenant not to sue the Releasees from demands, losses, or damages on account of any injury, including death or damage to property, caused, or alleged to be caused in whole or in part by the negligence of the Releasees or otherwise. I understand the Event may be photographed, videotaped, or otherwise recorded, and I agree to let the Releasees use my name, likeness, and demographic information free of charge in any manner and for any purpose in any media now known or hereafter developed. I am not under any legal disabilities that prevent me from being legally bound by this Waiver and Release of Liability for Personal Injury and Property Damage (this “Release”). I agree that this Release, and any dispute arising from or involving this Release, shall be governed by, and construed in accordance with the laws of the State of Arizona.*
