APPLICATION FOR MEMBERSHIP
- I hereby apply to become/ continue as a Real Tennis Member of the Cheltenham Recreation Club Limited;
- I agree to be bound by and to comply with the Memorandum, Articles and By-Laws of the Cheltenham Recreation Club Limited (available upon request and at the Club)
- I declare that I am over 18 years of age (parents should sign on behalf of Juniors)
- I declare that the details completed in this form are accurate and current including personal details, contact details and occupation in accordance with requirements from Liquor & Gaming NSW. If any of these details change in the future it is my responsibility to inform the Club of these changes to ensure all details remain accurate and current at all times
- I agree that my personal information may be collected and used by the Club for marketing purposes to improve the Club's services and to provide Members with the latest information about those services and any news related services and promotions. However, I can opt out at any time.

- This form will be submitted to the Club for review and approval
- Memberships run until 30 June each year, and subscriptions are based on the number of months (or part months) between the date of joining and 30 June. For example, someone who joins on 5 March, would pay for 4 months. The only exception is the Introductory membership category which is for three months from the date of their first game.
- Please pay for your membership separately by bank transfer to the Sydney Real Tennis (BSB 182-512 A/C 966790388) and quote "RT Member" AND your name
- Junior Member applications should be signed by a parent or guardian
