2026 Summer Camp Participation Agreement & Waiver

By signing below, I acknowledge that I have read, understand, and agree to all terms outlined in this registration form and at raiseleaders.org/terms. I authorize qualified RLMA, LLC (Raise Leaders Martial Arts) staff to administer first aid and CPR to my child if necessary and, if I cannot be reached, to arrange emergency medical treatment including ambulance transportation. I consent to the disclosure of relevant health information and to any medical evaluation, treatment, surgical procedure, or hospital care deemed immediately necessary by a licensed physician to safeguard my child’s health, and I accept full financial responsibility for such care. I understand that RLMA, LLC does not provide accident or health insurance coverage and that I am responsible for maintaining appropriate coverage for my child. I acknowledge that summer camp space is limited and reserved upon registration, that all payments are non-refundable and non-transferable, and that full payment is due prior to attendance. I authorize RLMA, LLC to process payments through Intuit QuickBooks, Square, and any payment processing platforms utilized by RLMA, LLC and agree to report any billing discrepancies within sixty (60) days or waive the right to dispute them. I grant permission for my child to participate in all martial arts instruction, recreational activities, leadership training, and approved off-site outings, including transportation when applicable. I understand that participation involves inherent risks including, but not limited to, falls, physical contact, sprains, fractures, and other potential injuries, and I voluntarily assume all such risks. To the fullest extent permitted by law, I release, waive, and discharge RLMA, LLC (Raise Leaders Martial Arts), its owners, officers, employees, directors, volunteers, independent contractors, and agents from any and all claims, liabilities, damages, losses, or causes of action arising from or related to my child’s participation, including those resulting from ordinary negligence, and I agree to indemnify and hold RLMA, LLC harmless from any claims brought on behalf of my child. I understand that Raise Leaders Martial Arts maintains a structured, respectful, and safety-focused environment and that behavior that is unsafe, disruptive, aggressive, or repeatedly non-compliant with staff direction may result in immediate dismissal from camp at the sole discretion of RLMA leadership without refund. I also grant permission for RLMA, LLC to photograph and record my child during camp activities and to use such media for promotional, marketing, educational, and social media purposes without compensation. This agreement applies to participation in the 2026 Summer Camp program and related activities between January 1, 2026 and January 1, 2027. By signing below, I acknowledge that I have carefully read and fully understand the 2026 Summer Camp Participation Agreement & Waiver, that I voluntarily agree to be legally bound by its terms, and that I understand I am waiving certain legal rights, including the right to bring certain claims, on behalf of myself and my child.
