New Client Application & Interview Questionnaire for ProDomme Services

Please fill out the following form in its completion if you are interested in Virtual or In-Person ProDomme Services from Leah Lorette, the Mistress Asherah & Domina of the Den of Venus. I will review your answers and reply to you via e-mail within 2-3 business days. This application acts as both a client safety screening, a test of your obedience and willingness to follow my directions, and a tool of determining if we are actually a good fit or not professionally. All information is kept encrypted and confidential.

Please answer the following questions to the best of your ability. Long-form answers are encouraged. Blank or short answers that show no effort will reflect poorly on your consideration as a Client. These questions help me to get to know you better, and to determine whether or not we would be a good fit.

If Local, Day Trip or Mid-Range and requesting in-person sessions, please note I only offer in-call (you come to me), unless otherwise discussed and approved. *If requesting out-call (I come to you), it must be at an approved 3 star+ hotel, and I bring an Assistant with me to the session.
Please provide your availability over the next two weeks, or within my posted tour window. I am available for bookings Friday evenings through Monday evenings, and only with at least 72 hours notice unless I am touring, then I require at least 24 hours notice for bookings.
Please provide multiple days and times that you would be available over the next two weeks. Time to sign waivers and review venue rules before a session, and 20-30 minutes of Aftercare after a session, is required. Please prepare to allow for 2 hours together for a one hour session booking, 3 hours for a 2 hour session, etc.

You understand that you may not solicit Mistress Asherah for sexual services or sexual intercourse of any kind, and that Mistress Asherah is not a FSSW and does not engage in genital play or contact, pegging, milking, oral or genital intercourse, or above the knee 'body worship' with any ProDomme client:
You understand that a nonrefundable deposit/retainer fee is required when booking all in-person sessions, with the remaining session fee due at the time of the session. Your deposit is transferable to another session in the case of a reschedule with at least 48hrs notice. Last-minute cancellations or no-shows will not transfer the deposit to a new session except for -if you're sick- [please ask to review our Deposit, Cancellation, and Sick Policy]... *All bookings except FMTY/Sponsored Tours require a $200 Deposit/Retainer Fee. *All virtual sessions must be paid for in-full at the time of booking prior to the Session... *Payments can be made via CashApp or in Cash.
You understand that Mistress Asherah does not discriminate but has the right to refuse to do a scene or session with any client and will not accept every Applicant as a client; has the right to end a session or scene if she feels uncomfortable, coerced, or unsafe at any point with no refund; and has the right to terminate a client relationship at any time for any reason she deems fit.
Health Intake

This is important information that I take into consideration when planning a session together just as much as your personal kinks or limits. The health and safety of my clients is of utmost importance to me, and I am both CPR and First-Aid Certified with a background in the medical, health and wellness field. I am disability-conscious, inclusive, trauma-informed, and COVID-careful in my practice and do my best to make scenes and sessions as accessible as possible for all clients. Your medical information is secure and confidential, and I adhere to Professional Client Confidentiality and HIPAA standards.
