Liability Release
I, the undersigned, understand that participating in the disc dog event organized by Aspire K9 Academy LLC involves certain risks and dangers to myself and my dog. These risks may include, but are not limited to, injury from dog bites, falls, collisions with other participants or animals, injuries to my dog, and the stress of competition.
I hereby waive, release, and discharge Aspire K9 Academy LLC, its officers, directors, employees, volunteers, agents, and successors from any and all claims, demands, losses, damages, and liabilities arising from any injury, death, or damage to property which I or my dog may suffer as a result of our participation in the event, whether such injury, death, or damage arises from the negligence of Aspire K9 Academy LLC or otherwise.
I agree to be solely responsible for the behavior of my dog during the event. I understand that my dog must be kept on a leash at all times when not competing and that I am responsible for controlling my dog and preventing any harm to other participants, dogs, horses, or property.
I acknowledge that there are horses on the property and that it is crucial for the safety of the horses and the participants to keep all dogs away from them. I agree to take all necessary precautions to ensure that my dog does not disturb or interact with the horses.
I certify that my dog is in good health and has been vaccinated according to the recommended schedule provided by a licensed veterinarian.
I agree to comply with all the rules and regulations of the event as established by Aspire K9 Academy LLC. I understand that failure to follow these rules may result in my expulsion from the event without any refund.
I hereby grant Aspire K9 Academy LLC the right to photograph, videotape, and/or record me and my dog and to use our likeness in all forms and media for advertising, trade, and any other lawful purposes.
I have read and fully understand the terms and conditions of this liability waiver and release form. I acknowledge that by signing this form, I am waiving my right to pursue litigation against Aspire K9 Academy LLC.
