THIS FORM SERVES FOR MAY 2024 TUMBLING REGISTRATION ONLY! SINCE THIS IS A MONTHLY PROGRAM ALL ATHLETES WILL HAVE TO COMPLETE A REGISTRATION FORM EACH MONTH TO REGISTER. A LINK WILL POSTED ON OUR WEBSITE AT THE END OF EACH MONTH TO REGISTER FOR THE NEXT MONTH. THE MONTHLY PRICE WILL BE DETERMINED BY THE NUMBER OF CLASSES AVAILABLE EACH MONTH!

Additonal Information
Please make sure your athlete does the following...
-Brings their own labeled water or sports drink.
-Dresses in athletic wear and tennis shoes.
-Hair is tied away from face or in pony tail.
-NO JEWELRY
COVID 19 PROCEDURES
• Athletes temperature will be taken upon entering the gym. Any athlete with a temperature at 99F or higher will not be allowed to enter the gym.
• There are 2 athletes allowed into the lobby at a time for check in.
• Check in will begin 10 minutes before the start of practice.
• Parents must remain in the parking lot at least 15min after the start of practice to make sure your athlete gets inside of the gym and checked in.
• Parents can remain in the parking area during the whole practice if they want to.
• Please arrive back to the gym at least 15min before the end of practice to pick up your athletes.
• Parents of athletes 9 years old or younger need to wait out front of the gym door, using social distancing, at the end of practice to pick up their athlete.
• Parents of athletes ages 10 and older need to be visible in the parking lot so your athlete can see you and your vehicle.
• A staff member will remain in the parking lot until all athletes are picked up.
• Any athlete that experiences any type of COVID 19 symptoms during practice will be sent home immediately.
• If any athlete experiences severe COVID symptoms during practice all athletes will be sent home for that day.
• Practice shoes will not be worn outside of the gym. Arrive in personal shoes and athletes will change once they get into the practice area.
• All shoes and bags will be sanitized during check in.
• Each athlete must have a bag to put their extra clothes in during practice. After they change, all items will be placed into the bag and placed into the cubbies. No items will be left on the floor.
• Athletes will use their cheer assigned book bag for Dynasty only. This bag should only consist of limited cover up clothing to get to and from the gym, along with cheer shoes, water or sports drinks and any practice attire.
• The water fountain is CLOSED. Your athletes must have their own water or sports drinks. The drinks will be stored in their book bags until break time and will be placed back into the bags after the break.
• Social distancing will be enforced as much as possible during practice.
• Coaches will only allow 3-4 athletes in the break area at a time.
• Mask will be worn at all times by all athletes, staff, and coaches while in the building.
• Athletes will sanitize and/or wash their hands throughout practice.
