Film Hub Midlands x BFI Film Academy 2024-25 - Young Person 16-25 Access to Activity Form

Guidance -
This form is only for approval by Ella Townsend - Young Talent Coordinator of Film Hub Midlands who oversees BFI Film Academy for 16-25's in the Midlands region.

Please carefully and thoroughly read the details below. 

1. Bursary applications or reimbursements must be signed and dated by both the attendee-participant and the lead contact of Film Academy in the Midlands, Ella Townsend before any claims can be made/paid. This is so we can forecast and allocate bursary spend as accurately as possible within limited resources.
2. Claims must be supported by receipts showing actual costs incurred. Any claims without receipt evidence will not be paid.
3. You must only invoice for the maximum offered at £20.00 maximum per day.
4. If you require financial support prior to attending an event or activity you must provide proof of purchase within 7-days of receipt of payment to the lead contact.
5. Attendee's-claimants must submit their reimbursement or bursary forms within 10-working days of attending the activity.
6. This form is for activity taking place between 1st April 2024 - 31st March 2025.

Claim Details - 

1. Please input the date, details, expense type, amount and tick the 'receipt uploaded' box to indicate you will upload the relevant receipt below.
2. There is a number 1 next to the 'Date incurred' input box.
3. Clearly write this number on the relevant receipt for this expense line.
4. You must do the same for Expense Line 2, 3, 4  e.t.c.
5. Click on 'Add Expense Line' to add your next expense.
6. Claims must be supported by receipts showing actual costs incurred, however if driving - mileage is at 0.45p per mile to and from location-venue of activity.
7. You must only invoice for the maximum offered at £20.00 maximum per day even if your train or car journey was £27.50 for example.
8. Remember - Film Hub Midlands will not be able to reimburse for any unaccounted/lost travel receipts.
9. Attendee's-claimants must submit their reimbursement or bursary forms within 10-working days of attending the activity.
