Volunteer Application Form

Thank you for your interest in Volunteering at United Mennonite Home. In order to be considered for our Volunteer team, you will need to sign an offence declaration form and apply for an up to date Vulnerable Sector Police Check. Once the Police Check has been completed and UMH has received the official copy, you will be reimbursed. Volunteers must be 14+ to apply. Volunteers under 18 years of age require to have a youth consent form signed by a parent/guardian. Once you apply, our Volunteer Coordinator will contact you to arrange an in person meeting to complete the orientation process.
