I am sigining that I would like to apply for Finacial Aid for UFA Metro Atlanta Grassroots.

I will apply on UFA Metro portal when I sign up for tryouts on May 1st. I will be required to pay $50.00 for application (refunded if your player does not make/accept a team spot)

Upon acceptance of the team I understand I have to pay $100 by August 1st

Then my account will be prorated anywhere from 25%-75% and will pay monthly fees

Your player / Parent will need to purchase a uniform from our uniform provider.

Financial Aid does not pay for tournamants or additional team fees.
