By submitting this completed application, I agree that:

1. I am 18 years of age or older.
2. The Caregiver has my permission to contact my veterinarian for a reference.
3. I will provide food, water, shelter, medical care, love, and affection for the natural lifetime of this cat, which could be as long as 20 years or more.
4. I agree to take this cat to a veterinarian at my own expense for periodic exams and vaccinations, as needed, and to obtain veterinary care should this cat become ill or injured.
5. I am not adopting this cat to be an outdoor cat.
6. I will not have this cat declawed.
7. I understand that the Caregiver makes no representations or warranties regarding the health, size, or temperament of the cat being adopted and that the Caregiver expressly declaims the same.
8. The Caregiver has my permission to inspect the premises under which the adopted cat is kept at any time to verify the cat's welfare.
9. I understand and accept that the Caregiver may remove the adopted cat from my possession  if they determine:  1) Conditions are unsuitable for the safety, health, or well-being of the adopted cat, 2) Food, water, and/or shelter provided for the adopted cat are inadequate, 3) I have falsified any information on this application; in addition, I understand and agree that I have no recourse of any kind resulting from such action by the Caregiver.
10. I understand and accept that this application remains the property of Coco's Heart Rescue.
11. I understand and accept that the adoption fee is nonrefundable.
12. All of the information that I have entered on this application is true to the best of my understanding and belief.
