Once this has been filled out, you will need to leave a $150 deposit on the next page. After that, you will be automatically put onto a waitlist and from then you will be contacted for more information.

      Deposit By signing this document, the client acknowledges that this deposit is non-refundable if the appointment is canceled or rescheduled within 48 hours of their scheduled appointment. Deposits are DISCOUNTED from the FINAL price of the FINAL session if said client has not canceled and has arrived at the specified time of their appointment.
