2026 All-State Player Nomination Form

Submit this form and upload ONE school photo (2x3 minimum size) of yourself. DO NOT USE A PHOTO OF YOU PLAYING IN A GAME. All photos become the property of OSCA. A copy of the information in this form will be sent to OSCA for use by the coaches to nominate you. The 2026 year nomination meetings will be held April 29, 2026 and this form must be submitted by April 22, 2024 in order to be eligible for nomination. Please PRINT your nomination form using your computer's server under the FILE in the top ribbon of the page.After printing the form, check your form for missing information or incorrect spellings and errors. PROVIDE your High School coach a copy of this nomination form.Once your High School coach has reviewed the form, select the SUBMIT button and your nomination form will be submitted to OSCA. THANKS! The DEADLINE for the form is April 22, 2026. All-State coaches and players will meet with teams on May 13,2026 for a photo and to discuss practices. East teams will meet at Union HS in Tulsa at 6:30pm and West teams will meet at PC North in OKC at 6:30pm. Players should wear their high school uniform for the photos.

By submitting this form I agree to attend the appropriate All-State match to be held at Catoosa HS in Catoosa, OK as follows: 6A, Thursday June 4th, girls play at 6pm, boys at 8pm; 5A, Friday June 5th, girls play at 6pm, boys at 8pm; 3A, Saturday June 6th, girls play at 1 pm, boys at 3 pm; 4A, Saturday June 6th, girls play at 6pm, boys at 8pm. Failure to attend the game will remove you from the All-State roster. If you cannot attend the game, inform your High School coach ASAP as soon as your receive the letter announcing your selection so a replacement player can be selected.
