Grooming Contract

 

Pet Liability

The Owner of the Pet shall be deemed the authorizing agent for all grooming instructions and procedures for the Pet, and shall be referred to as "Owner".

Medical Conditions & Medications

The Owner understands and acknowledges that grooming procedures can sometimes expose hidden medical problems as well as aggravate the existing ones. The Owner agrees to inform Booher's Doodle Farm and Grooming of any medical, physical, or emotional issues, allergies, sensitivities, or pre-existing conditions. These conditions may include, but are not limited to, prior surgeries, hip and/or joint issues, warts, moles, ear infections or skin problems. Because senior pets and pets with health problems have a greater risk of injury, these pets will be groomed for comfort and cleanliness, as Booher's Doodle Farm and Grooming will not compromise any pet's health and/or wellbeing to achieve "the perfect groom". Booher's Doodle Farm and Grooming will not be responsible for accidents or injuries to elderly or health compromised pets during their grooming. By signing below, the Owner agrees that Booher's Doodle Farm and Grooming has permission to groom their senior or health-compromised pet, and has discussed any and all (past and present) health problems to the best of their knowledge.

Current Vaccination Information

By signing this consent form, the Owner confirms that their pet is current on Rabies, Distemper/Parvo and Bordetella vaccinations. Proof of vaccination must be provided to Booher's Doole Farm and Grooming prior to grooming. It is the owners responsibility to provide updated vaccine records as vaccines are given. 

Aggressive Pets

By signing this consent form, the Owner agrees to inform Booher's Doodle Farm and Grooming staff of potential aggressive behavior towards people or animals that their pet may display. A muzzle will not used. Booher's Doodle Farm and Grooming reserves the right to refuse or stop services for aggressive pets at any time before or during the grooming procedure.

Accidental Injury

Although groomers use extreme caution and care in all situations, grooming equipment is sharp and there is always potential for accidents, including cuts, nicks, scrapes, and quicking of nails. By signing this consent form, the Owner confirms that he/she understands the risk of accidental injury to his/her pet and authorizes me to seek veterinary treatment at the cost to the Owner.

Fleas

If fleas are found on your pet, Booher's Doodle Farm and Grooming will give a one off flea treatment, Capstar, at an additional minimum cost of $15.00. This cost covers the medication and administration to ensure our grooming salon is free of active parasites. Capstar kills the fleas on the pet only. If there are fleas in the home area, the treatment will not protect your pet once they leave the establishment.

Mat Removal

The Owner agrees that Pets with matted coats require additional care and attention, as mats can be difficult to remove. The groomer will attempt to brush out mats, but severe matting may require the pet be shaved. Intense matting or brushing can cause dermatitis, irritation, and brush-burn, along with nicks or cuts. Booher's Doodle Farm and Grooming is not liable for any post grooming effects from this procedure. The Owner consents to mat removal procedures, and acknowledges risk to the Pet.

Pick Up Time

Booher's Doodle Farm and Grooming provides one on one grooming, your pet won't share his/her time with another pet. We feel one on one is best to reduce stress/anxiety and make your dog more relaxed with the grooming process. And the pet will receive more treats and hugs. We ask the owner to respect the next pets one on one time and pick up in a reasonable time after being notified your pet is finished. We do not crate while they are here.

Photographs

Your pet may be photographed before, during or after the groom. Photos may be used on various social media platforms or for educational purposes. Please inform Booher's Doodle Farm and Grooming if you prefer your pet not to be photographed.
