Financial Aid Application
This application is for players who would like to participate with Solano Surf Soccer Club and are requesting income-based financial aid.

Financial aid is intended to help families with demonstrated financial need. Assistance is limited and is awarded based on household income, family circumstances, available club resources, and completion of all required documentation.

Please complete the application below in full. Incomplete applications or missing documents may delay review or prevent your application from being considered.

If you are applying for financial aid for more than one player, you must submit a separate application for each player.

Before beginning the application, please have the following documents ready to upload:
- The two most recent pay stubs for every adult in your household
- The most recent W-2 forms for every adult in your household
- Any additional income documentation, if applicable, such as unemployment benefits, disability benefits, Social Security benefits, child support, self-employment income, or other household income
All information submitted will be kept confidential and used only to determine financial aid eligibility.

Submitting an application does not guarantee financial aid. Families will be notified after the application has been reviewed.
