Please download the "Verification of Acceptance to a New School" form. Fill out "Section 1" and give this form to your new school to fill out "Section 2" and return it to Academia.
It is the student's responsbility to submit the information requested by each school to complete their transfer. Students have a 60-day grace period to decide on a new school, and must begin the new school on the next available start date.

I understand that I have 60-days from the last day I attended Academia to decide on a new school and complete the transfer process. I understand that failure to do so will result in cancellation of my I-20 form and I will be Out-of-Status. This request cannot be cancelled.

Academia has explained the transfer process above to me, and I hereby sign to request the transfer of my I-20 form upon acceptance to my new school.
