I do hereby request and authorize Faces Clinical Skincare (hereinafter referred to as “Clinic”) to carry out the Elaine Brennan Skin Renewal Peeling Treatment (hereinafter referred to as “Skin Renewal Treatment”) upon me, in order to endeavour to improve my skin condition. The methods of treatment and their possible effects have been explained to me and are clearly understood by me. I am aware that during the treatment and for a short period thereafter, my skin will become sensitive, reddened and darker and there may be other side effects including, but not necessarily limited to the following:

1. For up to approximately 20 (twenty) minutes after the application of the Skin Renewal Treatment, I will probably feel a moderate burning sensation, giving way to a numb like sensation.
2. During the treatment, my skin will take on an appearance of dark and lighter patches of uneven coloring. However, the Clinic informs me that insofar as and to the extent that such condition is the result of the Skin Renewal Treatment and provided that it did not exist prior to the commencement of the treatment, it will disappear during the course of the treatment or shortly thereafter.
3. I acknowledge that an allergic reaction to the Skin Renewal Treatment is possible with accompanying symptoms such as redness, swelling and itching. The clinic has advised me that this allergy is not permanent or serious and can be adequately dealt with by the use of antihistamines.
4. If I have pimples on my skin prior to the commencement of the treatment, an increased breakout of the skin is to be expected as the Skin Renewal Treatment will help to bring to the surface the hidden infection in order to facilitate it’s removal.
5. The Skin Renewal Treatment takes longer to work on the skin around the eyes which said skin may accordingly appear dry for a few weeks after treatment. I am aware that in the case of darker skins, normalization will take longer than in the case of fair skins.
6. I have been advised that the normal effect of the said renewal treatment is that by noon of the third day, the skin will become dark, may swell and wrinkle on the surface giving rise to a tight pulling sensation over the entire treated area. This effect may cause me difficulty in eating, laughing or moving the face in other motions.
7. I am furthermore aware that the duration of the treatment will vary according to a number of factors, including but not limited to my skin’s reaction to the treatment, the area to be treated and the effect of the treatment.
8. I am aware that it is absolutely essential during the period of treatment, I should not apply any creams, lotions, ointments or any type of make-up or skin application of any nature whatsoever, except for those that the Clinic advises me.
9. I am aware that I am unable to wet, cleanse or wash my face, eyes and neck during the period of treatment. I understand that no water whatsoever may be used on or near my face and that I cannot wash my hair.
10. I am aware that it is absolutely essential that all verbal or printed instructions given to me by the Clinic should be adhered to by me strictly.
11. I am aware that shall I not follow those instructions, or do anything contrary to the matters referred to herein; I run the risk of damages and/or prejudice and/or disfigurement.
12. I am also aware that I should not spend any time exposing my skin to the sun during the period of the treatment and/or for a period of: Three months (3) subsequent to the termination of the treatment. I am also aware that during the peeling process when my skin begins to peel, I must not pull the skin. Doing so, will lead to skin irritation, pigmentation and blotches and in such event, the Skin Renewal Treatment may not be successful. I am aware that I must also not expose my skin the elements, sun, wind or cold upon the commencement of the peeling process.
13. I furthermore hold the Clinic, Elaine Brennan Skin Renewal Peeling and all of its employees, agents, representatives and servants, free and harmless in respect of any damaged and/or disfigurement and/or hurt and/or prejudice I may suffer as a result of the treatment and or its side or after effects. This indemnity and waiver and abandonment of my rights, shall apply in all and any circumstances and in relation to each and every act and/or omission committed by the Clinic including its reckless and/or negligent act or omissions.
14. I furthermore acknowledge that I am undertaking the treatment at my sole and absolute risk and that I will have no action of any nature whatsoever against the Clinic, Elaine Brennan Skin Renewal Peeling, its servants, agents, representatives or employees as a consequence of the treatment afforded me, whether arising from the treatment not being successful or having its desired effect, or otherwise, it being recorded that the Clinic give no undertakings, assurances, promises, inducements, warranties or guarantees and make no representations whatsoever in regard to the success of the treatment or any effects or results thereof.
15. The terms of the Agreement shall not be capable of being varied amended or cancelled whether in whole or in part unless such variation is accepted in writing by Elaine Brennan skin renewal peeling system.
16. I am aware that the Clinic, Elaine Brennan skin renewal peeling or its representatives may have made certain statements to me concerning the nature and effect of the Skin Renewal Treatment and other matters thereto. I acknowledge that any such statements whether made prior hereto, now or in the future or any other time and whether made to me verbally or in writing or whether contained in brochures, letters or any other documents furnished to me by the Clinic, are not intended to and do not have any legal effect and are not to be construed as guarantees, warranties, representations, stipulations or undertakings in regard to the treatment its nature or effects of any aspect thereof and I do not rely thereon in any event and I shall have no legal right or cause of action whatsoever arising out of any such statements. I further acknowledge that I have not been induced to undergo this treatment or enter into this agreement by any statement or representations whatsoever.
