FORM USAGE NOTES
The form should be pre-configured with all your details - see the light blue section below. Please review details, make any required changes, then click on the "Submit Nomination" button. There are two buttons which do the same thing, just click on one.
BOTH FIRST NAME AND LAST NAME MUST BE FILLED IN BEFORE NOMINATION BUTTONS BECOME AVAILABLE
Once you have done this, two things should happen
1. You will be taken to a second web page, which will tell you that the submission has been successful (or cancellation has been successful, if you select the CANCEL radio button below). Once you see this page, things have worked - close both the nomination and thank you pages.
2. Almost immediately, you should also receive an email confirming the submission or cancellation. If you don't receive this, check your email spam / junk folders, the email may be going there. If this happens, change the settings to allow email from MtCoolumVets and Jotform.
DON'T DELETE THE CONFIRMATION EMAIL. It will include a link back to your original submission. Click on the link if you wish to edit or cancel your original submission.
If you want to cancel an existing submission, click on the link in the confirmation email, select the CANCEL radio button and click SUBMIT NOMINATION button.
