Monthly Membership

I hereby acknowledge that I understand this contract. I likewise declare that I have full and legally binding authority to provide consent for Albert Macouzet DBA: Carlsbad Massage Reset to charge my personal debit or credit card monthly for 12 consecutive months from the start date,on the first of every month for $110.00.

I acknowledge the risk of any possible months that I dont use my membership that I assume the liability and responsibility of losing out on my FREE massage and possibly my paid massages if I have not used them by the time this contract ends.I understand my 2nd (second) massage of each month is my free one and if i dont use it that month = I LOSE IT.

It is my responsibility to schedule both my massages = ONLY through the website, every single month. No acceptions will be given.

I hereby authorize the Massage Reset to perform a one time debit each month with the card on file. In this regard, I understand that I maybe responsible for the one time charge of $110.00 @ the 12 months in the full amount of $1,320.00 (YEAR) if I fail to update this card on file for any reason that I may close this account, and not update "us" DBA: Massage Reset

I hereby express my permission for Carlsbad Massage Reset to use my card on file and charge it every month for the next 12 months.

The following has been explained to me and I acknowledge with full understanding:

I understand I can potentially lose my free massages given to me each month if I do not schedule accordingly. There is also potential for me to lose my paid massage if I do not use my PAID 1st (first) massage by the end of the calendar year. I also understand that I may not gift these to another person.*
