Media Release:
Girls Inc. of Greater Houston takes pictures/videos during our programs. As of August 1, 2021, we have an 'opt out policy on media release'. By signing this form, Media Release consent is implied unless you otherwise opt-out. If you do not want your participant's photo and or video take during our programs please provide a written statement requesting to opt your participant out of photo and or video by emailing programs@girlsinc-houston.org.
Liability Waiver Agreement:
As consideration for participation in these activities, I hereby agree that Girls Incorporated of Greater Houston and their respective employees, agents or contractors ("protected parties") shall have no liability for any claims, actions, demands, losses, costs, expenses, penalties, injuries, and/or damages of any kind arising out of or related to the program activities, even if such claims arise out of negligent, grossly negligent, reckless, or willful acts of the protected parties. I therefore waive any and all such claims that I and/or the child may have against the protected parties.
Notice of Surveys:
Girls Inc. of Greater Houston takes pride in having strong evaluation process in our programs to maintain quality and positive experience for participants. As part of this process we, along with our partners, utilize a variety of measurement tools.
In addition to this annually girls will be provided the opportunity to participate in our Girls Inc. National outcomes measurement survey. Our national surveys are conducted on an anonymous and secure online or paper format. By conducting these surveys, we are able to stay informed on the needs of our girls and the impact we have on our community.
Enrollment Acknowledgment:
Girls Inc. of Greater Houston is able to provide its programs and camps at no cost to families through generous donors and supporters, to ensure that families and girls are able to access our programs without financial barriers. By completing this application, you have applied to be a participant in our program, by signing below you understand completing this form does not guarantee enrollment. Any applicant not initially enrolled in our camps or programs upon enrollment will be placed on our waitlist.
By signing below you acknowledge that our program funding is often dependent on participant attendance, and when a participant receives enrollment in our programs they are taking up a spot that is no longer available to other applicants. By signing below you agree that you will be able to honor the time commitment of the program you are applying for, and agree that you will allow other applicants to take your spot if you feel like you are no longer able to do so.
Disclosure of Information:
Participants in Girls Inc. programs have the right to confidentiality and privacy. Girls Inc. will not share any of you or your child’s personal information unless you give us written permission to do so.
Notice of Exceptions to Confidentiality:
In instances where children inform us that they are being harmed by someone (physically, sexually, emotionally) or if they indicate that they want to harm themselves or someone else, Girls Inc. will break confidentiality.
CODE OF CONDUCT
BEHAVIOR
• I will treat other girls, staff members, and volunteers with respect.
• I will respect others’ property.
• I will respect others’ personal space.
• If there is a conflict with someone, I will try to solve it on my own.
• If I cannot solve the conflict by myself, I will find a Girls Inc. staff to help me.
TRANSPORTATION BEHAVIOR
Girls Inc. provides transportation for field trips during programming and designated camps and events. Girls Inc. staff are prohibited from transporting girls in their personal vehicles. During field trips, girls will follow these rules provided by Girls Inc. of Greater Houston:
• Listen to Girls Inc. staff when directions are given.
• Respect the driver and passengers.
• Always remain inside the vehicle; no hands, arms, or heads hanging out the windows.
• Wear seatbelts at all times, if applicable.
BULLYING PREVENTION
• Girls witnessing or experiencing bullying are encouraged to report the incident to a staff member.
• Girls can rely on Girls Inc. staff to promptly investigate each occurrence of bullying in a thorough and confidential manner.
• Any girl who engages in bullying may be subject to disciplinary action and possible removal from programming.
• If the bullied girl or the parent/caregiver feels that efforts made by Girls Inc. Staff were not appropriate in handling the situation, they should contact the Chief Executive Officer.
SAFETY
Parents/caregivers are required to provide Girls Inc. staff with all applicable phone numbers. This includes home, work, and cell phone numbers. We require that parents/caregivers also provide names and phone numbers of emergency contacts.
I hereby acknowledge that I have read, understood, and agree to the above waiver of liability. I, and my child, agree to follow the program rules and requirements established by Girls Inc. of Greater Houston. I agree to cooperate with staff to ensure my child will comply with the program and any partnering site policies while attending this program.
