We at Girls Inc of Greater Houston are inviting you all to join us for an exhilarating 2-week program called the Be BOLD Institute (BBI), where exciting discoveries, lifelong friendships, and boundless opportunities await!

Please note that thanks to our generous donors our programs are provided at no cost to support families who might otherwise not be able to afford the cost of our programs. There are a limited number of seats in each camp.

The form below is an application to be enrolled in the camp. Any applicant not initially enrolled in the camp will automatically be placed on our waitlist. 

 

Girls Inc. of Greater Houston is a small organization that provides services at no cost to our families.  We want to be inclusive of our participants' dietary restrictions, but we do not have the capacity at this time.  Our events are nut-free, and meals include a meat and a vegetarian option.  If you have concerns that your child can't or won't eat the food provided, please send them with a packed lunch and snacks for the day.  We appreciate your support while work to be more inclusive.

Attendance

All girls are encouraged to attend camp every day at Girls Inc. to receive the maximum benefit from our programs. Girls are expected to attend 80% of our camps to get the full Girls Inc. experience.

If a girl becomes ill prior to camp or if they are unable to attend under any circumstance,

please inform the Program Directors.

Sometimes, there are emergencies during camp hours and a girl may need to leave during camp. If that happens, please inform the Program Directors as soon as possible. If your child is unable to attend the remainder of camp due to illness or any other circumstance, please let the Program Directors know.

 

Drop-Off and Pick-Up

All parents/caregivers who drop off their child to camps or events, are required to provide proof of identification to Girls Inc. staff. Parents must pick up their child after the event or camp is over. Girls Inc. staff stay with children 45 minutes past the allotted time. If Girls Inc. staff are unable to reach the parent after the 45-minute timeframe, Girls Inc. is required to call Child Protective Services and report the incident for abandonment.

Authorized Pick-Up Person

For your girl’s safety, a state issued photo ID is required in order to release a child, and we will only release your child to individuals listed on the authorized pick-up list. Photo identification will be matched to the release information on your participant's registration form. It is the responsibility of the parent to include all possible pick up parties on your registration form to avoid any inconveniences. There will be no exception to this policy. 

We will NOT release members to anyone without the right credentials and that is not on the Authorized Pick-Up list. We cannot accept telephone verification for release. 

In order to protect your child’s safety, Girls Inc. reserves the right to not release a child to any person suspected of being under the influence of drugs or alcohol. In the event this should happen, law enforcement will be called immediately to further ensure the child’s safety.

Please note, any individual convicted of any crime against a child is absolutely prohibited from Girls Inc property, facility or Girls Inc related activities in or away from the center.  Failure to comply will result in immediate call to law enforcement.

To make changes to your authorized release list, please contact programs@girlsinc-houston.org. No exceptions.

Consent to Transport

I give permission for my child to be transported by Girls Inc. of Greater Houston. I understand that Girls Inc. of Greater Houston will transport my child to and from the various field trip locations. I release Girls Inc. of Greater Houston from all liability for any adverse results that may occur. I agree to release Girls Inc. of Greater Houston and its employees and officers from all liability with reference to the above- stated transportation.  

Media Release:

Girls Inc. of Greater Houston takes pictures/videos during our programs. As of August 1, 2021, we have an 'opt out policy on media release'. By signing this form, Media Release consent is implied unless you otherwise opt-out. If you do not want your participant's photo and or video take during our programs please provide a written statement requesting to opt your participant out of photo and or video by emailing programs@girlsinc-houston.org.

 
Liability Waiver Agreement:

As consideration for participation in these activities, I hereby agree that Girls Incorporated of Greater Houston and their respective employees, agents or contractors ("protected parties") shall have no liability for any claims, actions, demands, losses, costs, expenses, penalties, injuries, and/or damages of any kind arising out of or related to the program activities, even if such claims arise out of negligent, grossly negligent, reckless, or willful acts of the protected parties. I therefore waive any and all such claims that I and/or the child may have against the protected parties.

 

Notice of Surveys:

Girls Inc. of Greater Houston takes pride in having strong evaluation process in our programs to maintain quality and positive experience for participants. As part of this process we, along with our partners, utilize a variety of measurement tools.

In addition to this annually girls will be provided the opportunity to participate in our Girls Inc. National outcomes measurement survey. Our national surveys are conducted on an anonymous and secure online or paper format. By conducting these surveys, we are able to stay informed on the needs of our girls and the impact we have on our community.

 

Enrollment Acknowledgment: 

Girls Inc. of Greater Houston is able to provide its programs and camps at no cost to families through generous donors and supporters, to ensure that families and girls are able to access our programs without financial barriers. By completing this application, you have applied to be a participant in our program, by signing below you understand completing this form does not guarantee enrollment. Any applicant not initially enrolled in our camps or programs upon enrollment will be placed on our waitlist.

By signing below you acknowledge that our program funding is often dependent on participant attendance, and when a participant receives enrollment in our programs they are taking up a spot that is no longer available to other applicants. By signing below you agree that you will be able to honor the time commitment of the program you are applying for, and agree that you will allow other applicants to take your spot if you feel like you are no longer able to do so. 

Disclosure of Information: 

Participants in Girls Inc. programs have the right to confidentiality and privacy. Girls Inc. will not share any of you or your child’s personal information unless you give us written permission to do so.

 

Notice of Exceptions to Confidentiality:

In instances where children inform us that they are being harmed by someone (physically, sexually, emotionally) or if they indicate that they want to harm themselves or someone else, Girls Inc. will break confidentiality.

 

CODE OF CONDUCT

 

BEHAVIOR

• I will treat other girls, staff members, and volunteers with respect.

• I will respect others’ property.

• I will respect others’ personal space.

• If there is a conflict with someone, I will try to solve it on my own.

• If I cannot solve the conflict by myself, I will find a Girls Inc. staff to help me.

 

TRANSPORTATION BEHAVIOR

Girls Inc. provides transportation for field trips during programming and designated camps and events. Girls Inc. staff are prohibited from transporting girls in their personal vehicles. During field trips, girls will follow these rules provided by Girls Inc. of Greater Houston:

• Listen to Girls Inc. staff when directions are given.

• Respect the driver and passengers.

• Always remain inside the vehicle; no hands, arms, or heads hanging out the windows.

• Wear seatbelts at all times, if applicable.

 

BULLYING PREVENTION

• Girls witnessing or experiencing bullying are encouraged to report the incident to a staff member.

• Girls can rely on Girls Inc. staff to promptly investigate each occurrence of bullying in a thorough and confidential manner.

• Any girl who engages in bullying may be subject to disciplinary action and possible removal from programming.

• If the bullied girl or the parent/caregiver feels that efforts made by Girls Inc. Staff were not appropriate in handling the situation, they should contact the Chief Executive Officer.

 

SAFETY

Parents/caregivers are required to provide Girls Inc. staff with all applicable phone numbers. This includes home, work, and cell phone numbers. We require that parents/caregivers also provide names and phone numbers of emergency contacts.

 

 I hereby acknowledge that I have read, understood, and agree to the above waiver of liability. I, and my child, agree to follow the program rules and requirements established by Girls Inc. of Greater Houston. I agree to cooperate with staff to ensure my child will comply with the program and any partnering site policies while attending this program.

Coronavirus/COVID- 19 Liability

By registering, you acknowledge that an inherent risk of exposure to COVID-19 exists in any public place, including a camp/summit where people are present. By attending this event, you and any guests accept all risks related to exposure to COVID-19 and release the camp, summit, or other event from any liability for any illness or injury that comes from or is in any way associated with this event. Events follow CDC, state, local, and venue guidelines and rules. I agree not to attend any events if I feel ill or had recent exposure to a COVID-19 case.
