Consent Forms

Although camp staff have attempted to make the event as safe and comfortable as possible for all participants, participation in this camp could result in unforeseen injuries. Please review the forms below and provide digital signature for approval where indicated.
FOOD ALLERGIES & COMMITMENT FORM

Required for child to attend camp. 

Participants (Campers) of the event with food allergies need to be aware of such allergies and are responsible for self-care and possess knowledge about which foods they should not be around or ingest that could cause an allergic reaction.

Metro Tech staff cannot store medications of any kind for your child. Campers are allowed to bring medications and self-medicate, but we encourage dosages be given prior to attending camp. 

It is highly recommended that campers bring as little personal items as possible.  Do not bring back packs, as supplies are not needed. Cell phones should not be in use during camp time, but they can be used during break and at lunch. 

Camp staff may refer a camper to the office at any time if it is determined that a meeting with an administrator is necessary. If a camper is referred for disciplinary reasons, they will first be counseled. Continued or serious concerns will result in dismissal from the camp. Please note that tuition is non-refundable if a camper is dismissed for disciplinary reasons.

LIABILITY FORM 

Required for child to attend camp. 

In consideration of Metro Tech permitting my child to engage in all activities related to the camp, I hereby voluntarily assume all risks associated with participation in the camp and agree to release and waive any and all rights or claims for damages against Metro Tech, its agents and employees from any and all liability, including liability for negligence. 

I hereby acknowledge and state that my participation in this camp is entered into as a free and voluntary act with full and complete knowledge of the risks involved. In addition, in case of accident or need for medical attention, I give permission to Metro Tech’s staff to take named camper to a doctor and/or emergency facility. All expenses for treatment provided will be the responsibility of the parent or guardian but not Metro Tech.

PHOTO RELEASE

I grant Metro Technology Centers, operating as Metro Tech, permission to use any photographic, video, or audio image of myself or my minor in efforts by the technology center district to educate the public, promote career technology services, or in other ways deemed necessary.
