ETFC Summer STEM and Active Club Program Registration

STEM Camp – Ages 6-10 (Tuesdays 12:30 PM to 2:00 PM)

STEM camp, geared toward ages 6-10, will include fun, interactive activities and lessons based on science, technology, engineering, and math with projects to take home throughout the summer. One of the main focuses this summer will be nature.
Online registration is required to attend any programs through the summer months.
Registration will be open all summer long – online registration will be available throughout summer.
All summer art programs will be FREE.

Active Club ages 6-10 years old (Thursday 12:30 PM to 2:00 PM)

Active club will be geared towards ages 6-10 focusing on the physical side of enrichment. Body movement activities, games, and the importance of staying active for our minds and our bodies.
Athletics such as basketball, volleyball, soccer, tennis, yoga, dance and many more.
FUN body movement such as yoga, stretching, walking and jogging.
Online registration is required to attend any programs through the summer months.

Participation Waiver: I fully understand that the East Toledo Family Center, the City of Toledo or any other agencies affiliated with the East Toledo Family Center in producing the recreational activities are not responsible for any and/or all injuries that might occur during the class ortravel to or from the said program, and I do hereby release the East Toledo Family Center, its officers, agents, employees and all agencies affiliated with the recreational activities, either directly or indirectly, from the responsibility of injuries that may occur as the result of participation in the program.

Photo Release: I also give permission for my child to be photographed for promotional reasons by the press, Center staff or other parents.

Assumption of Risk and Waiver of Liability Relating to Coronavirus/COVID-19:
The novel coronavirus (“COVID-19”) has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, local governments and and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. East Toledo Family Center, of Toledo, Ohio has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, participation in volunteering at ETFC could increase your child(ren)’s or your risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and/or I (named above) may be exposed to or infected by COVID-19 by participating in recreational programs, including art, stem, active club, Elevate or any other programs held at the East Toledo Family Center and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the East Toledo Family Center may result from the actions, omissions, or negligence of myself and others, including, but not limited to, School employees, volunteers, and program participants and their families.
