​In consideration of using the facilities and services of RIVIERA HEALTH SPA (3601 Lomita Blvd, Torrance, CA 90505), I agree to the following:

​1. Health & Risk: I affirm I am in good health and assume all risks associated with the use of facilities (Steam Room, Sauna, Jacuzzi, Cold Bath, etc.) and services. I am solely responsible for any medical costs arising from my use of the spa.

2. ​Conduct & Safety: I will not use the facilities under the influence of alcohol or drugs. The Spa reserves the right to refuse service to anyone displaying harassing or inappropriate behavior.

 
3. ​Personal Property: The Spa is not responsible for any lost, stolen, or damaged personal items on the premises, including the parking lot.

4. ​Release of Liability: I hereby release and hold harmless Riviera Health Spa from any and all claims, including bodily injury, emotional distress, or sickness, arising from my use of the facilities or services.

​5. Entire Agreement: This document constitutes the entire agreement between Riviera Health Spa and myself.

​I have read and voluntarily sign this waiver, accepting full responsibility for any injuries or damages.
