AHAP Program Enrollment Form
  • Thank you for your interest in the Advanced Hearing Aid Professional Program.  This is Step 1 of the enrollment process. Once this form has been reviewed, you will receive an email for Step 2 to complete the process. Contact us at education@ihsinfo.org if you have any questions.

     

    IMPORTANT: The credential “AHAP,” the title “Advanced Hearing Aid Professional,” the program logos, and any other language that implies having earned or holding the credential may only be used in marketing by individuals who have completed the program, passed the final assessment, and maintain a license/certification/registration as a hearing aid professional in good standing.

    IHS reserves the right to revoke the credential and title if an individual fails to adhere to the Credential Use Policy and/or violates the AHAP Program Code of Conduct. At all times, learners and graduates must adhere to any applicable laws, rules, and regulations related to credentials, marketing, and scope of practice for their license.

    Your license must be in good standing and remain active throughout the program. Licensure will be verified by IHS staff.

    Refund/Cancellation Policy:

    Term 1 
    If a learner cancels their enrollment 14 or more days before the term begins, a refund of monies paid less a 10% processing fee will be issued. If a learner cancels their enrollment less than 14 days before the term begins, no refunds will be given. The last day that refunds will be issued for Term 1 is August 24, 2025. 
     
    Term 2 
    If a learner cancels their enrollment 30 or more days before the term begins, a refund of monies paid less a 10% processing fee will be issued. If a learner cancels their enrollment less than 30 days before the term begins, no refunds will be given. The last day that refunds will be issued for Term 2 is December 3, 2025. 
     
    No Show Policy 
    In the event a learner “no shows,” a refund will not be given. Attendance accommodations/allowances may be granted on a discretionary basis. The learner is encouraged to contact IHS as soon as a problem arises.
