  • Items or services donated will be used as a part of the 2025 Dream Gala Silent Auction, with all proceeds going to the Dream On 3 general fund. If future services are included in the donation, the commitment of service is between you the donor and the winning bidder. Signing this commitment form is a promise to uphold the stated terms of donated items and/or services. 

    This form serves as a tax receipt for your donation. Dream On 3 is a tax-exempt, nonprofit organization as described in section 501(c)(3) of the Internal Revenue Code. In accordance with IRS regulations, we confirm that no goods or services were provided to the donor in consideration for this contribution. EIN: 46-1988039.
