I declare that the medical history provided in the SYC 2025 registration form for First Name is correct. SYC Planning Committee Leaders have the right to sign for medical care and treatment as required on an emergency basis for the duration of the BC Summer Youth Celebration Youth Camp (held on August 24 to August 29, 2025 at Gardom Lake Bible Camp, on address 651 Glenmary Rd, Enderby BC, V0E 1V3) and its commute between the camp's pickup & dropoff location. I understand that BC-SYC (CNBC Westcoast) and Gardom Lake Bible Camp are not responsible for medical/dental costs or liabilities. I also agree that BC-SYC (CNBC Westcoast) and Gardom Lake Bible Camp will not be liable for any damages in the event of any accident or misfortune to the participant or personal property.
Media Release Agreement: I agree to permit reasonable use of photos and videos of me taken in this camp for the purpose of promoting the SYC ministry. *Please note that media including photos and videos from the camp will only be shared within the participants of SYC2025.*

Refund Policy: If notification is received 14 days prior to date of camp starting, then you will get the registration fee less $60. No refund will be issued within 14 days of camp start date. No refund will be issued for arriving late or leaving early. Refunds for emergency reasons will be decided on a case by case basis.*

Rules and Expectations for Participants: if the participant violates the rules and expectations of the camp, you agree that we will contact the youth's legal guardians (if you are a under 19 years of age) to arrange for early dismissal from the event.*
