Veterinary Release
I agree and have read through to the following statements in case of an emergency at Launa's leads.

In the event my dog needs emergency veterinary care due to an accident or illness I permit Launa's leads to seek emergency veterinary care for my dog If every effort has been to made to contact me but I was unavailable to return contact.

I understand Launa's leads works hard to prevent accidents and injuries and that such problems may occur no matter how well we care for your dog.

I understand that efforts will be made to seek veterinary care from my vet but if this is not possible I give permission for an alternative veterinary practice to treat my dog. I also permit the alternative vet to have access to my dog's medical records.

I give my permission for my dog to be safely transported to the veterinary clinic or for on-site care to be provided.

I (dogs owner) agree that I have full responsibility for the payment incurred by any vet bills and I agree that launasleads will be not liable for any payments needed from the vet.

I understand Launa's leads will not be held liable for any injury or loss and is released from any responsibility related to transportation, treatments given, or any costs that may arise.

We love to take photos during our walks and share them on our social media channels. We like to share your dog's experience with us and will also communicate this on social media with your permission.

By signing this release form you are permitting us to share your dog's photos and experience on social media and for use in our marketing and future promotions.

We completely understand and respect your decision if you would prefer us not to use photos of your dog online.
I hereby grant Launa's leads permission to use photographs and videos of my dog during their walks in publications newsletters social media and online, including publications and marketing.

Off lead consent form
Please make sure you read our off-lead/leash terms carefully & agree to each point. If you do not agree then we will be unable to let your dog off lead.

I give consent for Launa's leads to allow my dog off lead in a secure environment where:

1 - there is no threat to their safety.

2 - My dog has good recall and will return to walker when called upon.

3 - Any incidences or accidents that result from my dog being off lead will be my responsibility (owner). I relinquish Launa's leads of any libability for accidents that may occur due to dog being of lead.

4 - If my dog needs vet care as a result of being off lead i agree as the dog owner i will be liable to cover all expenses occurred,/ and that launasleads won't be liable for any expenses or held accountable.

5- If Launa's leads feels my dog is not suitable to be off lead, I agree for my dog to be walked on their lead or a long line.

6 - My dog shows no signs of aggression towards any other animal, human or livestock.

I agree with all of the above statements and am happy for my dog to be allowed off lead.
