Submit your entry by May 10, 2024 through the official submission form. If your entry is selected, you will be required to complete a phone interview about your story and an in-person video interview at an Adelante location.

This promotion is open to all current Adelante patients. Patients who are under 18, must be accompanied by a parent or guardian, who will be included in the interview. Interview participants agree that their name and/or image may be used by Adelante online, on social media or on any other promotional materials. The total promotional award is $100 dollars, in the form of a gift card to the store of your choice, and will only be awarded once the steps above are completed.
