Round 1 Submit application for 2024/2025 season

Round 2 Succsessful applicants from Round 1 will be notified via email with a time to attend a company class for in person meet/greet, parent information session

Students will be required to attend
- All Ballet Technique classes at MDS (electives maybe studied if their is no conflict with ballet schedule
- All SYB weekly company class
- All SYB company rehersals
- All SYB performances
- Previose ballet training is not required if the student is passionate
2024 season show dates - TBA

Rehearsals/Company class will be on Saturdays 3-5.30pm at 1/12 Old Great Northern Hwy Midland. Please note occasional Sunday afternoons maybe needed on occasion.

Principals, Soloists parts will be auditioned in company class.

I Name and Surname (must be parent/guardian/dancer over 18yrs) * I recognize and understand the risks of physical injury inherent to ballet, fitness, performing arts, acrobatics and dance training and I fully assume these risks. I hereby release Swan Youth Ballet Company Inc, its affiliates, Midland dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold
harmless Swan Youth Ballet Inc, its affiliate Midland dance studios, event sponsors, employees and dance teachers for liabilities, costs and judgments arising from acts of omissions committed by myself or my child which result in injury or damage to any person or property. I will pay any medical costs that may be attendant as a result of injury to me or my child.

I have fully informed myself of the content of this waiver, all terms & conditions policy’s, procedures, fees and charges contained within.

Further I hold Swan Youth Ballet Company Inc harmless in agreement by reading it all before I sign this waiver, and I understand that I sign this document in my own free will; no oral representations, statements, or inducements, apart from the written statement, have been made.
