Waiver and Release of Liability
I am allowing my companion animal to receive massage and/or Reiki therapy by Misty Denny of Misty Downs Animal Wellness & Training. I understand that massage and Reiki are not a substitute for medical treatment or medications, and that it is recommended that I work with my Veterinarian for any medical conditions that my animal may have. I understand that any of the massage or Reiki sessions are for the purpose of stress reduction, relief from muscular tension, general relaxation and improvement of circulation and range of motion.
I understand that the Massage Therapist/Reiki Practitioner cannot diagnose illness or disease, cannot prescribe medications. I understand that any information provided by the Massage Therapist/Reiki Practitioner is for educational purposes only, and is not diagnostically prescriptive in nature.
I have informed the Massage Therapist/Reiki Practitioner of all my animal’s known physical conditions, limitations, medical conditions and medications. It is my responsibility to update this information with the Therapist/Practitioner and contact my Veterinarian if my animal’s physical condition, limitations, medical condition or medications should change.
By signing this release, I hereby agree to indemnify and hold harmless Misty Downs Animal Wellness & Training, its employees, owners and agents from any claims, or claims by any member of my family or any other person accompanying me to any massage/Reiki session or surrounding area thereto as a result of any action by any dog, including my own from all liability past, present and future, relating to massage, Reiki and bodywork.
NOTICE OF INHERENT RISKS WITH HORSES – Please Do Not feed, pet or let your dog’s interact with the horses:
I understand Equines have the propensity to behave in ways that may result in injury, harm or death to persons on or around the equine; have unpredictable reactions to such things as sounds, sudden movement and unfamiliar objects, persons or other animals; are susceptible to certain hazards such as surface or subsurface conditions, collisions with other equines or objects; propensities include kicking, biting, stamping, stumbling, rearing, and others; tack equipment can fail resulting in falling or loss of control; and activities have the potential of a participant to act in a negligent manner that may contribute to injury to the participant or others, such as failing to maintain control over the equine or not acting within the participant’s ability. Equine activities are INHERENTLY DANGEROUS.
