By submitting this form I hereby consent to the storing, processing and transferring all or part of the visual and audio output I have submitted while filling this Application Form for use in advertising and promotions for The DARK KARNIVAL belonging to GLADIATOR WARZ. As well I hereby consent to the use of my image and likeness being utilized and taken at The DARK KARNIVAL event for future promotional purposes for The DARK KARNIVAL event and other events hosted by GLADIATOR WARZ. I also agree to arrive at the latest at the event site by 7pm central time on Thursday 9/18/25. If one is unable to do so then this will need to be addressed on a case-by-case basis directly with Khaos & GLADIATOR WARZ prior to the event. Purely for scheduling purposes.

*Gas assistance negotiable upon request and is judged on a case by case basis.
By submitting this application and upon receipt, verification, and approval via Khaos of Gladiator Warz the submitter willl be entitled to compensation packages detailed forthwith:

LOOT PACK FOR THE DOOMED
Access wrist band for entry to the DARK KARNIVAL
Gate swag & Ticket
(1) meal per day - (1) evening meal on Friday and Saturday (1) breakfast on Sunday
(3) professional shots provided by photographer TK Wanderlust
Options provided to purchase more shots after the event

Gas assistance available on a case by case basis

If enough passes are sold for the event there will be a monetary bonus applied to all entertianment.
